Redshift differs from Amazon's other hosted database offering, Amazon RDS, in its ability to handle analytic workloads on big data data sets stored by a column-oriented DBMS principle. The following seven factors help assess the mortality risk:ġ. Cardiologists who performed the angiographies and echocardiographs were blinded to the TIMI scores of patients. Echocardiography study was done by an expert cardiologist at the time of admission using vivid ® S6 GE device. Stenosis of the coronary artery was given a score of 0 if narrowing was less than 50% and a score of 1 if stenosis was greater than 50%. TIMI score predictive factors (Presence of at least three risk factors for coronary artery disease, age of 65 years or older, presence of 2 or more episodes of angina 24 hours before the presentation, aspirin use in the past seven days, previous history of coronary stenosis of 50% or more, ST-segment deviations greater than or equal to 0.05 mV on initial ECG on admission, elevated serum cardiac markers of necrosis) and angiographic data were also included in the checklist.Ĭoronary artery angiography was done and reported by one expert cardiologist. Patients with unclear past medical history or past coronary angiographic history, prior coronary artery bypass graft (CABG), atypical chest pain, evidence of ST-segment elevation in the initial ECG, those who underwent the angiographic procedure due to reasons other than ischemic heart disease, and those who did not give consent for participating were excluded.ĭata on baseline characteristics, including demographics (age, sex), risk factors (weight, hypertension, diabetes, current smoking, family history of coronary artery disease), and medical history (prior angina or MI), were collected. Patients with UA or NSTEMI who underwent angiographic study were included in the study using census sampling method. Written informed consent was obtained from all the patients for participating in the study. The study was approved by the ethics committee of Shahid Beheshti University of Medical Sciences (Code: IR.1398) and the researchers adhered to the principles of the Helsinki Declaration and patient data confidentiality throughout the study. This prospective cross-sectional study was designed to evaluate the correlation between TIMI score, and the number of vessels involved in the angiographic study of NSTEMI and UA patients, presenting to Modarres Hospital, Tehran, Iran, from April 2019 to August 2020. This study aimed to evaluate the correlation between TIMI risk score and the number of vessels involved in the angiographic study of patients presenting to emergency department following UA or NSTEMI. This is particularly important for lower-middle-income countries, which are increasingly affected by cardiovascular disease epidemic and encompass different genetics and lifestyle. Hence, the development of the TIMI risk score originated from developed countries, with limited data evaluating the effectiveness in developing countries. The TIMI score was established as one of the most commonly utilized risk assessment models in the chest pain units to warrant further workup. Patients presenting with Unstable Angina (UA) or NSTEMI that fit score 3 or more on the TIMI model are mostly recommended to undergo early invasive management with cardiac angiography and revascularization if necessary ( 9). There are seven components used in the calculation of the TIMI score. The TIMI (Thrombolysis in Myocardial Infarction) research group has introduced a specific model, the TIMI risk score assessment tool, which has been found to be predictive of the severity of vascular diseases and the potential of coronary circulation involvement in chest pain patients ( 8). Nonetheless, electrocardiogram (ECG), as the most readily available diagnostic tool in chest pain units, is not adequately helpful in decision making( 7). Choosing the best treatment in the initial stages following quick diagnosis is of great importance in improving the outcome. Several randomized clinical trials performed in the past two decades have established that immediate and complete restoration of flow in the occluded artery decreases infarct size, improves survival rates, and preserves left ventricular (LV) function ( 5, 6). Each year, three million people experience ST-segment elevation MI (STEMI) and also non-ST-segment elevation MI (NSTEMI) was estimated to occur in about four million ( 4). Myocardial infarction (MI) is known as the most severe presentation of CAD and CAD accounts for 30% of all mortalities ( 3). Despite recent developments, coronary artery disease (CAD) remains the leading cause of death across the world. Advancements of cardiac care units and revascularization methods, as well as the developments in pharmacotherapy, have led to improved patient outcomes after acute coronary syndrome (ACS) ( 1, 2). Call stacks for the same code with the monitoring tool present on the left are on the right: In the image below, call stacks for slightly modified code without the monitoring tool are on the left. Fortunately, there wasn’t any other SQL Server work occurring during my testing so PerfView was quite effective at showing the difference between the two servers. I was able to temporarily add third party monitoring to the server with the fast code. Common choices are Windows Performance Recorder or PerfView. However, how can we really prove that the monitoring tool is the culprit? Where exactly is that extra CPU time going?ĮTW tracing can provide a summary of call stacks during a sampled time period. Disabling the monitoring tool made both CPU times match. It was running on the server with the slower code and it wasn’t running on the server with the faster code. There was a fair amount of circumstantial evidence that the monitoring tool was responsible. That doesn’t mean that we shouldn’t use actual plans as performance diagnosis tool! Digging In In some cases, simply asking for a SQL Server actual plan can more than double the execution time of a query. Different types of monitoring will have different overheads for different types of work within SQL Server. Again, we can’t conclude that the vendor is doing something wrong. In my case, the culprit ended up being a popular SQL Server third party monitoring solution (I will decline to name the vendor). With that said, it’s definitely suspicious and not something that you want to unexpectedly see. It does not necessarily mean that there’s some kind of severe issue with the server that takes more CPU time to perform the same work. To be clear, the only thing that you can safely conclude in this kind of situation is that there’s some kind of unknown configuration difference between the two servers. I definitely wouldn’t expect an 8X difference for two servers that were supposed to have identical performance! There are of course a lot of factors in play here, but I would expect a healthy machine to take somewhere around 200-800 ms of CPU work. The first server that I tested took about 2400 CPU ms to execute the code. Otherwise, there would be no reason to write a blog post. Going back to the intro, you can probably guess that I did not see identical CPU times when comparing the two servers. Paul White explains why the ASYNC_NETWORK_IO waits appear when the same code is run as part of a batch in this stack exchange answer. A temporary stored procedure is used to avoid ASYNC_NETWORK_IO waits. It’s designed to spend as little time on waits and latches as possible. The code is extremely efficient (from a certain point of view) and CPU time will closely match elapsed time as as long as there’s no other processes on the lucky CPU that runs my beautiful code. Sometimes I get a spider sense while working with virtualized SQL Server instances and want to check for problems. For example, I might be switching from old hardware to new hardware and I want to immediately see a faster query to know that I got my money’s worth. Sometimes I have a need to run a quick CPU comparison test between two different SQL Server instances. ![]() With some pergola designs, you can pick from a variety of hues to suit your garden color schemes. You can find step-by-step advice on how to build a pergola in our guide. The amount of slope needed depends on the size of your structure (the larger the roof, the higher the angle will need to be) and also the weather conditions of your region.Īs mentioned, some designs have cleverly integrated drainage systems already installed, so whether you pick an angled roof or not is more dependent on your preferred style. This will prevent rainwater from pooling in the middle of your pergola and leading to potential damage, as instead it will help it to drain away. However, if you have a permanent solid roof on top of your design, or one that can close to create a solid surface, then you may want to opt for a slight angle by increasing the height of the structure at one end. Traditional pergolas tend to be more or less flat at the top (at a 90 degree angle from the posts). But, if you've combined the structure with climbers, it will provide an additional support for a fuller display. It's a subtle but certainly charming design feature which looks good as is.
